The subject of the order is "Successive deliveries of reconstruction implants, adhesives and consumables for the needs of the Operating Theater of the General Provincial Hospital in Kielce".

• Annex No. 3 to the SWZ - draft provisions of the public procurement contract on a deposit basis, applies to Package No. 5
• Annex No. 3a to the SWZ - draft provisions of the public procurement contract on a deposit basis, applies to Package No. 1, 2, 3, 4.
• Annex No. 3b to the SWZ - draft provisions of the public procurement contract regarding the subject of lending apply to Package No. 1, 2, 3, 4, 5
